In 2024, SFHRC started a Community Health Worker (CHW) program. A CHW is a frontline public health worker who connects their community with healthcare, housing and social services. They are often members of the community they serve, which allows them to build trust and understand the unique needs of the people they assist. CHWs provide various services like health education, outreach, advocacy, and informal counseling.
Bridging the gap:
CHWs act as liaisons between communities and healthcare systems, helping individuals access necessary services and resources.
Health education and promotion:
They educate community members on various health topics, promote healthy behaviors, and encourage preventive care.
Advocacy and support:
CHWs advocate for their clients' needs, connecting them with resources like food assistance, housing, and transportation.
Outreach and engagement:
They conduct outreach activities, organize community events, and build relationships within the community.
Cultural competency:
CHWs are often culturally sensitive and aware of the specific needs and challenges of the communities they serve.
Examples of CHW activities:
Providing screenings and referrals.
Helping individuals complete applications for housing or social service programs.
Facilitating communication between patients and other service providers.
Offering informal counseling and support.
Organizing support groups and educational workshops.
Conducting home visits to provide support and assess needs.
Community Workers facilitate community development initiatives and collective solutions within a community to address issues, needs and problems associated with recreational, health, housing, employment and other welfare matters.
